1. Assumption of Risk

Training at BAM BAM involves inherent risk of physical injury, illness, or property damage — including from strength training, plyometrics, speed and agility work, conditioning, and the use of training equipment. I voluntarily assume all such risks, known and unknown.

2. Release of Liability

I release BAM BAM Training LLC, its owners, employees, trainers, contractors, and affiliates (the “Released Parties”) from all claims arising from my participation, including those caused by their ordinary negligence. This release does not cover gross negligence, recklessness, or willful misconduct, and does not waive any right that cannot be waived under California law.

3. Indemnification

I will indemnify and hold harmless the Released Parties from claims, damages, or expenses resulting from my participation, except those arising from their gross negligence or willful misconduct.

4. Medical

I am physically fit to participate and have consulted a medical professional about any health concerns. I have disclosed all relevant injuries, conditions, allergies, and medications, and will report any changes. I authorize BAM BAM to summon emergency medical care and accept financial responsibility for it.

5. Participants Under 18

If the participant is under 18, the parent or legal guardian signing below has full authority to do so and agrees to sections 1–4 both on the minor’s behalf and individually, including indemnifying the Released Parties against any claim brought by or on behalf of the minor.

6. Governing Law

This agreement is governed by California law, with venue in Sonoma County.

7. Acknowledgment & Signature

I have read and understood this agreement, had the opportunity to ask questions, and agree voluntarily. I understand I am giving up substantial legal rights. If any provision is unenforceable, the rest remains in effect. Typing my name and submitting this form is my signature and has the same effect as signing by hand.

• I authorize BAM BAM to arrange emergency medical care.
